Home Physiotherapy Services

Foundations Physio has been created as a community based service to support residents in Surrey and Hampshire. All of our team have extensive experience working from people’s homes and we pride ourselves on being both knowledgeable and approachable. Our aim to create a community of people and services focussed on providing both great care and a support network for those with a disability.

 

Whilst home visits can be convenient we understand they can also be a source of anxiety and you may have many questions, our initial session aims to put you at ease, introduce you to our team/service and explore your goals and aspirations. We can then discuss a plan moving forwards with you and your family in order to begin your rehabilitation journey.

Our expert physiotherapists offer assessment and treatment sessions in the comfort of your own home or care facility. Seeing you at home allows us to understand you, your family and what is important to you in order to better tailor your rehabilitation.

How Our Home Physiotherapy Service Works

After gathering some initial information, our office team will arrange a time with you for one of our expert physiotherapists to visit you at home for an assessment. Your physiotherapist will go through the history of your condition, a physical assessment and then discuss your goals of treatment. We aim to leave you with some practical advice, information and a treatment plan at the end of the assessment and will arrange a time for a follow up visit at a time and day to suit you.
 
We are delighted to be able to offer a range of services, from exercise classes and intensive outpatient physiotherapy to hydrotherapy and we can discuss all of these options with you during your time with us.

What to Expect from Your First Home-Visit Physiotherapy Session

Your first home visit will mostly be about getting to know you, what’s important to you and what you would like to achieve through physiotherapy. We know how difficult a change in health or mobility can be and all of our team are experienced in supporting people through their own individual journey. We take a person-centred approach and realise that everyone we work with is different and not defined by their condition.
 
Our sessions are based on providing empathetic support combined with practical advice on physical activity that will best suit your personal circumstances. All of our team have a passion for helping those with neurological conditions or complex disability move more, live well and get back to doing what they enjoy most.

Do you bring equipment to a home visit?

Yes. Our physiotherapists bring portable equipment that can be used in your home environment. We also adapt exercises to your living space, and if needed, we can recommend walking aids or home adaptations to support your rehabilitation.

Absolutely. We regularly provide physiotherapy within residential and nursing homes across Surrey and Hampshire. We work closely with care staff and families to ensure joined-up, consistent care.

Most home visits last 45–60 minutes, depending on your needs. Initial assessments are usually around 1 hour, as they include a full discussion of your medical history, a physical assessment, and goal setting.

Where possible, yes. Continuity is important, so we aim to keep you with the same physiotherapist for your sessions. This helps build trust, ensures consistency, and allows your therapist to track progress closely.
